Tournament Request Form (Ennismore Minor Hockey)

Coaches and/or managers are to use this form to request travel permits for their upcoming tournaments. All of the information required can be found on the tournament listing. You can either upload a screenshot of the tournament listing or answer the following questions. Team name & contact info will be required either way to submit the form. If you have any questions, please direct them to our OMHA Rep Londa Junkin at [email protected] If the tournament is outside of the OMHA district EMHA will be reaching out to the team contact to request a payment method to pay for the necessary permit(s).
