EMHA members are invited to submit a team staff evaluation, including evaluations for Head Coach, Assistant Coaches, Trainers, and Managers. Please note that your response is anonymous, unless you choose to leave your contact info at the bottom. The executive will not respond to allegations made on the evaluation form.
