All on-ice volunteers in our IP program (U5–U9) must complete this volunteer application form to participate in any on-ice activities. This form is required before the start of each season.
Please be prepared to provide your Hockey Canada Registration number, and ensure your Vulnerable Sector Check is current and up to date.
DO NOT COMPLETE THIS FORM IF YOU ARE A ROSTERED COACH OR TRAINER FOR AN IP TEAM IN THE CURRENT SEASON.
