All on-ice volunteers must complete this application form prior to participating in any on-ice activities each season. The final deadline to sign-up to become an on-ice volunteer is December 10th of each season.
Please be prepared to provide your Hockey Canada Registration number, and ensure your Vulnerable Sector Check is current and up to date.
