This form is used to gather the information our OMHA Representativ, needs to complete the Permission to Skate (PTS) form (AAA tryouts, all levels) and/or the Non-Resident Passport (NRP) form (AA or A tryouts for U13 and above). Permission-To-Skate (PTS) forms will be emailed to those requesting when available and completed. When you receive the PTS,
you must print it and submit it at the AAA tryout registration desk
(normally, at the first tryout) before your child will be permitted to step on the ice.
I understand that the PTS form will be sent to me by email before AAA tryouts begin. I understand that my child will not be permitted to skate at AAA Zone tryouts unless I submit a printed copy of the PTS form to the AAA tryout registration officials.

Non-Resident Passports
MUST
be picked up @ 2591 Chemong Road- you will receive an email when the form is ready for pick up. You will collect it from the black mailbox on the fence.
I understand that I need to pick up the NRP before AA tryouts begin. I understand that my child will not be permitted to skate at AA tryouts without the NRP.
